Team OCHRI saves critically ill sickle cell diseased 18-year-old medical student

Recently an 18-year-old female 1st year MBBS Student and resident of Nagpur was shifted to Orange City Hospital & Research Institute- a unit of Ravi Nair Hospitals Private Limited, Nagpur on 24.10.2019 during early morning in a very critical condition from another Nagpur based Critical Care hospital with Bilateral Extensive Pneumonia with Renal Failure with Severe Thrombocytopenia. She had persistent complaints of fever and breathlessness and her health was deteriorating, hence her father who is serving as Police Inspector shifted her to OCHRI as per advise of senior police official. One examination, her General condition was not satisfactory and oxygen saturation was very low. Immediately She was put on Bipap support with other allied supportive treatment. She was admitted under care of Dr. Rajesh Atal- Senior Critical Care Physician. Battery of investigations was done in which she was found to have Sickle Cell Disease. She was diagnosed first time as a Sickle Cell Disease at the age of 18 yrs. She was not having any history of Sickle Cell related illness in the past.  This patient had a prolonged stay in ICU and was managed with Bipap Support, Hemodialysis and other supportive treatment protocol. After prolonged stay and a very stormy course she was successfully discharged from hospital. Parents of this patient thanked and appreciated the effort of treating doctors and OCHRI.

Dr. Rajesh Atal informed that, “She was diagnosed Sickle Cell Disease at the age of 18 years which is an unusual thing to occur, because usually Sickle cell is diagnosed in childhood itself. This patient has no symptoms of Sickle Cell till date and was admitted first time with Sickle Cell Crisis with related complications. The cornerstones of treatment involve the management of painful vaso-occlusive crises, hemolytic anemia, and disease complications as well as prevention of infection. Allogeneic bone marrow transplantation is the only curative treatment option.

Dr.Anup Marar- OCHRI Director added that Sickle cell disease Sickle cell syndromes are hereditary hemoglobinopathies in which red blood cells are not shaped as they should be. Red blood cells usually look like round discs. But in sickle cell disease, they’re shaped like crescent moons. Although Neonatal screening for sickle cell anemia has been implemented across the U.S., it is still not made compulsory in India. It should be made compulsory in areas and communities where SCD is rampantly observed.

Dr.Vidya Nair- RNHPL Director complimented Dr. S.J. Acharya, Dr. Amit Pasari- Nephrologists, Dr. Sushant Muley – Pulmonologist and Dr. Ashwini Tayade- Infectious Disease Specialist who supported Dr. Rajesh Atal for successfully treating this patient. Government of India led by Shri.Nitin Gadkari, Union Minister of Micro, Small and Medium Enterprises is promoting inclusion of indigenous service industries under MSME wherein they are covered under Micro, Small and Medium Enterprises Development (MSMED) Act 2006 for related engrained statutory benefits.

In view of OCHRI’s comprehensive Polytrauma care management, OCHRI is the only hospital of Nagpur to have been recognized by Roadways, Railways and Airport Authority of India for aiding them in emergency care. OCHRI is the only hospital to have been recognized by 60 major organizations of Central India.  OCHRI is the only Private Hospital of Nagpur whose services are availed by Government authorities as an emergency care center during visit of any Z Plus security covered VVIP to Nagpur. Highlighting this registration, Shri.Simha Chalam, OCHRI Executive Director- (Finance) stated that in accordance with the provision of Micro, Small & Medium Enterprises Development (MSMED) Act 2006; Micro, Small and Medium Enterprises (MSME) are classified into Manufacturing Enterprises & Service Enterprises. It provides first-ever legal framework for recognition of the concept of “enterprise” which comprises both manufacturing and service entities.

Dr. Anup Marar- OCHRI Director added that although Government of India provides lots of benefits to MSME so as to promote Make in India and Skill India; many service industries are not registered under MSME since it is voluntary. All service industries should check with their CA’s and get registration so as to avail the embedded benefits boosted by Government of India led by Visionary Prime Minister Shri.Narendra Modi.

Shri.Udaybhaskar Nair- Nairsons Chairman mentioned that apart from Priority Credit clearance and Lending; Less interest rates, Income Tax exemption, Credit Linked Guarantee Scheme, Market assistance, varied subsidies, technical support, grants for infrastructure development and Technology/ quality upgradation support to MSMEs are few of the benefits.

Rare case of Adrenal Gland Tuberculosis diagnosed and treated at OCHRI

In September 2018, one 55-year-old female from Satna, MP was admitted in Nagpur based Orange City Hospital & Research Institute a unit of Ravi Nair Hospitals Private Limited under care of Dr. Vina Bang- OCHRI Physician. She presented with vague symptoms of vomiting, generalized weakness, weight loss, appetite loss, hyper pigmentation and low grade fever. She was evaluated extensively. All possible causes were ruled out. Clinically she had all signs of adrenal insufficiency so imaging was done with special focus on both adrenals. She had granulomatous lesion in Lt. Adrenal Glands leading to its size up to 3 to 4 times the normal size. Rt. Adrenal Gland was also enlarged. Her Monteux test was positive and ESR was raised. She was started on ATT (Anti Tubercular Treatment) after counselling and explaining to patient & relatives about nature and course of disease. She was discharged in couple of days and they continued treatment and followed up in OPD since then. Before coming to OCHRI; she had been taken to many private hospitals where she was evaluated but no confirmed diagnosis was made. Because of all these medical problems she also suffered from depression and was also being treated by Psychiatrist.

In first two months; patient showed very slow recovery. Her fever subsided. But she had persistent hypoglycemia, postural hypotension, hyper pigmentation and there was no weight gain also, which were suggestive of Adrenal Insufficiency. So Dr. Vina Bang started steroids along with ATT.

Dr. Vina Bang informed that, “Recently, she completed her ATT with good compliance to treatment. Gradually steroids were also tapered off. Now since 3 months she is off steroids. Her repeat scan showed resolution in adrenal lesion and reduction in size of Glands. She has gained weight significantly from 44 kg to now 64 kg. There is no further episode of fever in last 1 year. Her appetite has improved and pigmentation has also faded. The most important outcome is patient is very happy.

Dr. Nishikant Lokhande- OCHRI Radiologist mentioned that adrenal tuberculosis can result in a life-threatening disorder called primary adrenal insufficiency (PAI) due to the destruction of adrenal cortex. CT forms the mainstay of evaluation due to its high spatial resolution and availability, but MRI also has a known role in assessing adrenal lesions, particularly in young patients where radiation dose is a concern.

Dr. Anup Marar- OCHRI Director added that corticosteroids should be used cautiously with antitubercular therapy in view of drug interaction seen between them. In cases of adrenal crisis, corticosteroid replacement becomes mandatory. The comprehensive methodical assessment undertaken by Dr. Vina Bang before initiating both treatment protocols is appreciable.

Dr. Usha Nair- RNHPL Director complimented Dr. Vina Bang for the detailed intricate evaluation which helped her to clinch the diagnosis and provide solace to this MP patient who reached OCHRI as last hope. She added that it is total faith of relatives on treating team which gives them liberty to think out of box and treat the patient successfully.

Relatives of said patient thanked Dr. Vina Bang and TEAM OCHRI for successful treatment of their patient.
